2. Print and sign health waiver form (link below)

3. Send health waiver along with payment to the JCHYK address:

You will receive a confirmation email when the above steps are completed

REGISTRATION FEES

Early bird available until July 15th 

First child: $300 Each additional child: $275

 

After July 15th 

$325 per child
